* Name and Contact details of the Sports Official
* Parent / Guardian information (if the Official is under 18)
* Level
* Governing Body
* Preferred Categories
* Preferred Travel Distance
* Affiliation, DBS and Safeguarding expiration dates (if know)
* A list of games & Events that the Official has been on in the past
